- You reside in the Senior Whole Health Medicare Complete Care (HMO D-SNP) geographic service area
- – and – you must have both Medicare Part A and Medicare Part B
- – and – you must be eligible for Medicare and Full Medicaid
- – and – you must be enrolled in our Medicare Advantage program.
