Community Choices Waiver

The Community Choices Waiver can help individuals remain at home and avoid unnecessary or premature nursing home placement. The waiver is available for persons who are unable to perform activities of daily living such as bathing, dressing, and toileting due to illness or disability.

Personal Care
This service provides assistance with general household activities. This service also helps individuals with activities such as bathing, dressing, preparing meals, housekeeping, and observing health signs.

Attendant Care
This is a member directed service to provide personal care assistance.

Companion
This service provides short-term relief for caregivers and needed supervision of members.

Adult Day Health Care (ADHC)
This service provides medically supervised care and services provided at a licensed day care center. Transportation to and from the home is provided within 15-miles of the center.

ADHC Nursing
This service provides limited skilled nursing procedures as ordered by a physician that may be provided by the ADHC nurse at the center.

Case management and coordination of this waiver service
A Molina Care Coordinator will be assigned to help you manage your health care needs.

Minor home adaptations
This service involves both pest control services and minor physical adaptation to the home.

Home Delivered Meals
This service provides regular or special diet meals delivered to the member's home.

Nutritional Supplements
This service provides some nutritional supplements.

Specialized Medical Equipment and Supplies
This service provides medical supplies to assist with care at home.

Transportation to adult day health care
This service is a daytime community-based program for adults not living in Supported Living Facilities.

Personal Emergency Response System (PERS)
This service provides an electronic device which enables high risk individuals to secure help in the event of an emergency.

Institutional Respite Care
This service provides temporary relief for the caregiver by admitting the member to an inpatient facility (nursing home or hospital).
