When you become a member of Molina Healthcare, you’ll get a member ID card in the mail. Everyone in your household who is a member will have their own card.

Make sure to check that the information on your card is correct. This helps you get the care you need. 

Your member ID card has important information. It shows your member ID number, how to contact your doctor, and important phone numbers for getting help from Molina.

 

Don't forget your ID cards

  • You need to carry your Member ID card with you to all doctor visits, when you pick up medicine or go to the hospital. This card helps your doctors, nurses, and pharmacists know if you can get health care services from Molina. 

  • Don't forget to bring your Medicaid ID card too! It’s important to make sure you get the services you need.

 
What do I do if I lose my member ID card?

  • Go to the My Molina member portal, and print a copy of your member ID card, or call Member Services at (855) 882-3901 (TTY: 711) to ask to have a new card sent to you at no cost.
