Medicaid Managed Care (MMC) Health Plan
Medicaid Managed Care is a NY sponsored health insurance program for eligible children and adults with low income. Affinity by Molina Healthcare's MMC plan offers comprehensive, free or low-cost health coverage for eligible residents. Molina Healthcare PLUS Health Plan
Molina Healthcare PLUS is a special program for current and eligible Affinity Medicaid members over the age of 21. Molina Healthcare PLUS provides Medicaid members with their health care, plus care for behavioral health such as mental health, substance use disorders and rehabilitation.

Child Health Plus (CHPlus) Health Plan
Child Health Plus (CHPlus) is a free or low-cost health insurance plan for eligible children who are under the age of 19. Children under the age of 19 who are not eligible for Medicaid and who have limited or no health insurance may be eligible for CHPlus. If your family income is high, you may still qualify for CHPlus. Essential Plan
The Essential Plan is offered to individuals who are ineligible for QHP, Medicaid or Child Health Plus and don’t have access to employer based coverage. Essential Plan covers doctor visits, tests ordered by your provider, prescription drugs, hospital care—the same essential benefits as other health plans. But it costs much less!
