Liberty Dental®  provides dental services to Affinity PLUS Members through their network of contracted providers. Children under the age of 21 are covered for diagnostic, preventive and restorative services. Adults have limited benefits such as exams, x-rays and cleanings.
If you need dental services, you must go to a dentist who accepts Liberty Dental®. For more information call Liberty Dental® at (866) 609-1184 (TTY:711).

You may have to pay for services that are not covered. You may also have to pay for services from providers not part of our network. If the services were an emergency, you don’t have to pay. If you need help, call Member Services.
