Getting your Molina rewards is easy! You can now submit your information through your My Molina® member portal account. Molina's reloadable reward card lets you continue earning rewards for completing important wellness visits and screenings. Shop at participating retailers in person or online at Walmart.com; use for in-store purchases or other purposes such as bill pay or pet supplies. Use the Store Finder at
HealthyBenefitsPlus.com/MolinaHealthyRewards to find stores where you can buy eligible items.

Most rewards are claimed online or by phone, while others are earned automatically by enrolling in Perx to track certain medications. Look for the icon to see which rewards require enrolling in Perx.

Claiming your rewards is easy!

  1. Complete one or more of the health screening(s) mentioned above (excluding Perx rewards).
    2. Note: To earn a reward(s), you must have Molina Healthcare of New York as your primary insurance at the time service was given.
  2. Log in to your My Molina member portal account.
  3. Click on the 'My Wellness' tab, then click the 'My Healthy Rewards' tab.
  4. Click on the 'Request Submission' button to view the secure online form.
  5. Fill out the form with details of the health screening(s) you have received. You can submit multiple screenings at once.
    6. Note: Rewards can be submitted under either an individual's My Molina® member portal account or under the primary insurer's account. If you do not have an account, you will need to create one or call Molina Member Services at 1.800.223.7242, TTY: 711 for help claiming your reward further.
  6. Click the 'Submit' button, and you're done!

Important information:

Each reward can be received no more than once a year. For example, a dental visit can be completed every six months, but you can only receive one $25 reward each year. The only exception is for Viral Load Suppression, which can be claimed up to three times a year.

Requests for rewards can be submitted up until March 31 of the following year. Submissions received after the deadline will not be processed.
