Your vision care is provided to you by Affinity by Molina Healthcare through Superior Vision. Superior Vision is a national vision benefits manager. How do members get care?

If you need vision care, you must go to an Affinity network provider. To find the vision care center nearest you, call (800) 223-7242, TTY: 711. You may also view our Provider Directory.

You may have to pay for services that are not covered. You may also have to pay for services from providers not part of our network. If the services were an emergency, you don’t have to pay. If you need help, call Member Services.
