Children in Custody: Important Contact Information

Primary Care Provider (PCP)
The name and phone number of your child’s PCP is on their Molina member ID card. A PCP is your child’s primary doctor. Regular visits to your child’s PCP will make sure your child is growing up healthy.

Molina Care Manager
Every child in foster care can access Care Management services tailored to their individual needs. You can contact your child’s Care Manager by calling Member Services. Call (800) 642-4168 (TTY 711) Monday to Friday from 7 a.m. to 8 p.m.

OhioRISE 
Your child may qualify for the OhioRISE specialized managed care program. To contact OhioRISE, call (833) 711-0773  Monday to Friday from 7 a.m. to 8 p.m. Learn more about OhioRISE.

Nurse Advice Line
You can speak to a nurse any time, day or night. Nurses are ready to answer your health questions or give advice on where you can take your child to get the care he or she needs. Call (888) 275-8750 or for TTY call (866) 735-2929 any time. Learn more.

Transportation
Your child gets 30 extra one-way trips each year at no cost. This benefit can be used to go to the doctor, dentist, pharmacy or other medical and health resources. Call (866) 642-9279 (TTY 711) at least 2 business days before your visit to schedule transportation. Learn more about your transportation benefit.
