Your Member Identification (ID) Card

When you become a member, you get a Member Identification (ID) card in the mail from Molina MyCare Ohio Medicaid.

Your Molina MyCare Ohio Medicaid ID card will now be your ID card for Medicaid.

Make sure the information on your card is correct. If your information is not correct, call Member Services at 855-687-7862 (TTY 711) from Monday to Friday, 8 a.m. to 8 p.m. local time. We will send you a new member ID card.

You should receive your Member Handbook and ID card before your effective enrollment date (the date your healthcare coverage starts). You will get 1 permanent ID card in the mail. You will not get a monthly card or yearly card with Molina MyCare Ohio Medicaid.

Be sure to carry your member ID card with you at all times. You must show your member ID card to get covered healthcare services and prescriptions drugs.

Your member ID card has important phone numbers on it:

  • Member Services
  • 24-hour Nurse Advice Line
  • Your primary care provider's (PCP) phone number

 

If you lose your member ID card:

  • View or print a temporary ID card on MyMolina.com or on the My Molina app.
  • Ask us to send you a new card on MyMolina.com or by calling Member Services.

If your phone number, email address, or mailing address change, you need to tell your county caseworker. Call your local County Department of Job and Family Services (CDJFS) office to talk to your caseworker. You can find the phone number for your local CDJFS office here.
