As a
Molina Complete Care for MyCare Ohio (HMO D-SNP) member, your Medicare and Medicaid benefits are set up so you get the most out of your plan!

Molina Complete Care for MyCare Ohio offers a range of services under this plan! View your Member Handbook for full benefits and details.

Dental
We offer dental benefits that include preventive and comprehensive dental services such as cleanings, fillings, and dentures. The services listed below are examples of covered services, and each has a limit. Limits might include maximum allowance, number of procedures allowed, or frequency of services.

  • Routine cleanings
  • Fluoride
  • Fillings
  • Deep cleanings
  • Crowns and bridges
  • Extractions
  • Root canals
  • Dentures
  • Anesthesia

 

You also get extra dental benefits:

  • Extra dental checkups to keep your teeth healthy.
  • Up to $6,000 each year for dental treatments like fillings, crowns and dentures.

 

Did you know? Most general dentists can perform medically necessary specialty procedures such as deep cleanings, root canals, and extractions. Check with your usual dentist first if you need one of these procedures. 

Fitness
The Silver&Fit® program offers you access to participating fitness centers and/or a Home Fitness Kit.

If you have a caregiver, you can bring them with you to help you use the fitness center’s services and equipment.

There are two ways to start using your Silver&Fit® benefit:

  • Go to SilverandFit.com. After registering on the website, you can find fitness centers, pick a Home Fitness Kit, and search for online classes.
  • Call Molina Member Services at (855) 665-4623 or TTY 711, Monday to Friday, 8 a.m. to 8 p.m., local time.

 

Molina Complete Care Card (a supplemental benefit)
You get $230 each month on a prepaid card to pay for over-the-counter (OTC) medicines. If you qualify, you can also use the card to pay for needs like transportation, groceries, and utilities.*

Over-the-Counter (OTC) with your Molina Complete Care Card
You can get non-prescription OTC health and wellness items like vitamins, sunscreen, pain relievers, cough and cold medicine and bandages. 

You can use your Molina Complete Care Card:

  • In Store: Through participating retail locations.
  • Online: Visit NationsOTC.com/Molina
  • By Phone: Call (877) 208-9243 (TTY: 711) to speak with a NationsOTC Member Experience Advisor, 24 hours a day, 7 days a week, 365 days a year.
  • By Mail: Fill out and return the OTC Order Form in the OTC Product Catalog.

 

Things to remember:

 

Transportation
You can get a ride to your health care appointments and services. Get a ride to office visits, physical therapy, dialysis and the pharmacy. If you have recurring appointments, these can be scheduled ahead of time.

Things to remember:

  • Call 48 hours before your appointment to schedule transportation.
  • If you need to cancel your transportation service, call 24 hours before your appointment.
  • If you need special help such as wheelchair accessibility, tell us that when scheduling transportation. We want to make sure the correct transportation can be arranged.
  • You will need prior authorization if you travel over 100 miles for a one-way trip.

 

Extra transportation as a Molina Complete Care for MyCare Ohio member.
You get up to 104 one-way trips each  year to get to health resources or go to the doctor.

  • Use your extra trips to go to the doctor, dentist, eye exam, grocery store, food bank, WIC appointments, SSI, health education visits, and CDJFS (County Department of Jobs and Family Services) appointments.
  • You have flexible options like all-day bus passes, taxi or van services and mileage reimbursement to cover approved trips.
  • You can also request rideshare services like Uber or Lyft.

 

Vision
Your eyes are the tools to see the world! It is important to keep them safe and healthy. We offer vision care services such as eye exams, eyeglasses, and contacts. We want to make sure you see clearly and stay well so you don’t miss a single moment with your loved ones. 

Your extra vision benefits include:

  • $0 copay for an annual eye exam
  • $300 each year to buy glasses, contact lenses, or special features like tinting.

 

IMPORTANT: If you opt to receive vision care services or vision care materials that are not covered benefits under this plan, a participating vision care provider may charge you his or her normal fee for such services or materials. Prior to providing you with vision care services or vision care materials that are not covered benefits, the vision care provider will provide you with an estimated cost for each service or material upon your request. You can only be billed by your vision care provider if you agree to pay for the service and sign a written statement before getting the service. If you get a bill from a provider you did not agree to pay, call Member Services.

Hearing

Your extra hearing benefits include:

  • $0 hearing tests every year.
  • Hearing aids covered every two years.
  • Over-the-counter hearing aids for mild to moderate hearing loss.

 

More extras for your health and wellbeing

Social Connections program:

  • Talk or chat with someone in our Compassionate Call Center any time.
  • $0 smartphone and phone plan to help you stay connected, for qualifying members.
  • Technology Coaches to help you use your phone or computer to access healthcare providers and benefit information.

 

Extras for caregivers:

  • 8 one-way rides to visit a member in the hospital or nursing home.
  • Online support groups, helpful videos, and tips on caregiving topics.
  • Care Coaches for personal advice and support by phone.
  • On-demand caregiver certification program for expert training.
  • 40 extra hours of respite care
