About Waiver Services
MyCare Ohio enrollees who were receiving waiver services through PASSPORT, Choices, Assisted Living, Ohio Home Care and Transitions II Carve-Out waivers are now covered in the new MyCare Ohio combined waiver. If you are receiving waiver services, you are automatically enrolled in the MyCare Ohio waiver.

As a Molina Dual Options MyCare Ohio member, your services are coordinated and authorized through your care manager. Waiver service planning is still to be based on medical necessity and services are coordinated to meet your specific needs.

You can stay with your current providers on the MyCare Ohio waiver. The transition period, between 90 and 365 days depending on the service, allows for members to continue receiving services from their current waiver providers. After the transition period, your provider can continue to serve you if he or she joins the Molina Healthcare provider network. To learn more you can read your Member Handbook.

Molina Healthcare will keep your services and providers the same. Changes will not occur unless:

  • There is a significant change in your condition, living arrangement, or caregiver status; or
  • You express a desire to direct your own services through the Choices Waiver Program; or
  • The transition period ends after 365 days from the time you enroll in your Molina Dual Options plan

Through the MyCare Ohio waiver, Molina Dual Options MyCare Ohio members will have access to expanded waiver services. As a Dual Options member, your services are coordinated and authorized through your care manager. Waiver service planning will continue to be based on medical necessity and services will be coordinated to meet your specific needs.

For more information contact your local County Department of Jobs and Family Services, or call Molina Member Services.

Patient Liability
Because you are covered by Medicaid, you usually pay nothing for covered services. However, you may have to pay a “patient liability.” Patient liability is a cost you may have to pay for some long-term care services. Learn more.
